Client Eligibility Criteria


The eligibility requirements for the Home Delivered Meals program are as follows:

  • How do I get Meals on Wheels or other in-home services that are available in the City of Chicago? In-home services provided to homebound seniors include Meals on Wheels, chore/housekeeping services, adult day care programs, and respite care.   • What age must one be eligible for CDoSS services?
    Most CDoSS programs are available to those 60 years of age or older. The employment services are open to those 55 years and older. The Information & Assistance services are available to all ages and can be accessed by calling 312-744-4016 or 312-744-6777(TTY)

The eligibility requirements for the HomeMod program are as follows:

  • Chicago residents with disabilities under 60 years of age
  • Must be head of household, spouse or legal dependant of the head of household
  • Households with incomes below 80% of the area median income may qualify for a full grant; those whose income is over 80% the area median income may qualify for a partial grant
  • Homeowners and renters may qualify for this program
